This page is for non Local Government Pension Scheme (LGPS) administrators and independent financial advisors. It’s not for use by LGPS members or employers.

You’ll find our transfer discharge forms attached to this page.

Choosing the right form

Please use the appropriate discharge form based on the receiving scheme and the type of benefit being transferred:

Deferred refund

If the member is transferring a deferred refund, these are the form options:

  • Qualifying recognised overseas pension scheme (QROPS): Form 1
  • Previously contracted in occupational pension scheme: Form 2
  • Personal pension: Form 3
  • Previously contracted out occupational scheme: Form 4

Deferred benefits

If the member is transferring deferred benefits, these are the form options::

  • Qualifying recognised overseas pension scheme (QROPS): Form 5
  • Previously contracted in occupational pension scheme: Form 6
  • Personal pension: Form 7
  • Previously contracted out occupational scheme: Form 8

Additional voluntary contribution (AVC) for a member who has vested

If the member is transferring an AVC, these are the form options::

  • Qualifying recognised overseas pension scheme (QROPS): Form 9
  • Previously contracted in occupational pension scheme: Form 10
  • Personal pension: Form 11
  • Previously contracted out occupational scheme: Form 12

Pension credit

If a pension credit member is transferring their main scheme benefits, these are the form options:

  • Qualifying recognised overseas pension scheme (QROPS): Form 13
  • Previously contracted in occupational pension scheme: Form 14
  • Personal pension: Form 15
  • Previously contracted out occupational scheme: Form 16

Additional voluntary contribution (AVC) for pension credit member

If a pension credit member is transferring their AVC, these are the form options:

  • Qualifying recognised overseas pension scheme (QROPS): Form 17
  • Previously contracted in occupational pension scheme: Form 18
  • Personal pension: Form 19
  • Previously contracted out occupational scheme: Form 20

References to “contracted out” refer to a scheme's status before April 2016.
